These forms are to be completed after  the student has had a free trial lesson with the teacher.

1) Registration Form/Demographics 
2) Student Weekly Schedule Form: Please list all activities the students participates in during the week. The purpose of this form is to find the most convenient time for all students to have their piano lessons.
